Abstract: 

Purpose: To present potential benefits as well as limitations of premium intraocular lens (IOL) use, and provide insight in future of premium cataract surgery. Methods: Bibliographic research was performed in PubMed/Medline database, and the most recently updated papers were evaluated. Keywords used were: premium intraocular lens, Multifocal intraocular lens, toric intraocular lens, toric Multifocal intraocular lens, accommodative intraocular lens, and the respective brand names. Results: Multifocal IOLs provide uncorrected distance visual acuity (UDVA) of 0. 03 logMAR in 82. 3%e95. 7% of patients and overall spectacle independence in 81%e85% of patients. Toric IOLs provide UDVA of 0. 3 logMAR in 70%e95% of patients, residual astigmatism of 1 D or less is noted in 67%e88% of patients, and spectacle independence is reported in 60%e85% of patients. Toric Multifocal IOLs provide UDVA of 0. 3 logMAR in 92%e97% of patients, and spectacle independence is reported in 79%e90% of patients. Accommodative IOLs represent intensively developing field in ophthalmology, and the results are still variable depending on the IOL model. Conclusions: Premium IOL technology and advanced surgical techniques have significantly improved postoperative visual outcomes. Future developments will potentiate development of new premium IOL designs that will provide spectacle independence and excellent visual outcomes after cataract surgery.

Abstract: 

Purpose: To evaluate vision-related quality of life in two sets of patients after routine cataract surgery implanting with traditional versus Multifocal intraocular lens (IOLs). Methods: In a cross-sectional prospective study, 58 and 33 candidates for cataract surgery were divided into traditional (Acrysof SN60WF, Alcon Laboratories, Inc) and Multifocal IOL (AcrySof IQ PanOptix IOL TFNT00, Alcon Laboratories, Inc. ) groups, respectively. The primary outcome was VFQ-25 scores. The secondary outcomes were making comparisons between the two IOL types in the near vision and the driving items. Results: The mean patients’,age in traditional and Multifocal IOL groups was 60. 85 ±,7. 40 (55% female) and 59. 85 ±,8. 95 (36% female) years, respectively. The mean VFQ-25 total scores in traditional and Multifocal IOL groups before and after surgery were 63. 69 ±,4. 95 and 72. 15 ±,9. 66, and 98. 08 ±,0. 70 and 95. 70 ±,1. 30, respectively (P = 0. 001 & 0. 001). The mean scores of night driving in traditional and Multifocal IOL groups were 38. 79 ±,20. 50 and 44. 35 ±,21. 12 (P = 0. 1) before surgery which improved to 97. 41 ±,7. 68 and 56. 45 ±,11. 12 after surgery, respectively (P = 0. 001). The mean scores of near vision in traditional and Multifocal IOL groups were 46. 83 ±,10. 56 and 50. 53 ±,8. 58 (P = 0. 2) before surgery which improved to 89. 94 ±,4. 87 and 100. 00 ±,0. 00 after surgery, respectively (P = 0. 001). Conclusion: Vision-related quality of life after cataract surgery with either type of traditional or Multifocal (PanOptix) IOLs improved to an excellent level. Traditional IOLs provided more satisfaction in nighttime driving while Multifocal IOLs provided increased satisfaction in near and intermediate vision.

Abstract: 

Purpose: To compare AcrySof IQ and Sensar intraocular lens (IOLs) in terms of spherical aberration and contrast sensitivity.Methods: Thirty-four eyes of 34 patients undergoing phacoemulsification cataract surgery were randomly assigned for implantation of AcrySof IQ or Sensar IOLs. Three months postoperatively, bestcorrected visual acuity (BCVA), spherical aberration (with 4 and 6 mm pupil diameters) and contrast sensitivity under photopic and mesopic conditions at spatial frequencies of 1, 2, 5, 10 and 20 cycles per degree (cpd) were determined.Results: Patients included 21 male (61.8%) and 13 female (38.2%) subjects. Mean BCVA was 0.083±0.071 logMAR in the IQ group and 0.145±0.097 logMAR in the Sensar group (P=0.079). Spherical aberration with both 4 and 6 mm pupil diameters in the IQ group was less than in the Sensar group (0.183±0.107 mm vs 0.354±0.138mm, P<0.001 and 0.26±0.096mm vs 0.435±0.152mm, p=0.002; respectively). Contrast  sensitivity was higher in the IQ group at 1, 2, 5, 10 and 20 cpd under photopic conditions and at 1, 10 and 20 cpd under mesopic conditions (P<0.02).Conclusion: AcrySof IQ aspheric IOL seems to have better visual outcomes regarding spherical aberration and contrast sensitivity as compared to spheric Sensar IOL.

Abstract: 

Purpose: To compare visual quality in patients receiving Alcon AcrySof ReSTOR Multifocal versus AcrySof SA60AT monofocal intraocular lenses (IOL).Methods: In this interventional study, patients with senile cataract undergoing surgery were enrolled. The age of the patients ranged between 40 and 85 years, and their potential preoperative vision was 20/30 or better. In all patients phacoemulsification was performed through a temporal 2.8 mm incision. Postoperative visual acuity (VA), refraction, and contrast sensitivity (with and without glare) tests were done at 1 and 3 months. A total of 101 eyes were evaluated. A Multifocal IOL was implanted in 52.5% of cases, and in the rest, a monofocal IOL was used.Results: At 3 months, the mean distant VA without and with correction was 0.11 and 0.04 logMAR, respectively, in the Multifocal IOL group, and 0.14 and 0.03 logMAR, respectively, in the monofocal IOL group (P>0.10). The near VA without and with correction was 0.14 and 0.05 logMAR, respectively, in the Multifocal group and 0.22 and 0.04 logMAR, respectively, in the monofocal IOL group, statistically significantly better in the Multifocal group (P=0.038). At 3 months after surgery, contrast sensitivity with and without glare showed statistically significant inter-group differences at 6, 12 and 18 cycles per degree (CPD), and at 6 and 12 CPD, respectively. These figures were higher in monofocal group.Conclusion: Use of Multifocal IOLs in cataract surgery can restore near vision to some extent in addition to distant vision. Contrast sensitivity in recipients of Multifocal IOLs is lower than those with monofocal IOLs.

Abstract: 

In this case report we introduce 9 years old boy who had suffered recurrent bone infections including the sternum, clavicles, navicular bone, distal radius and proximal tibia since the age of 5. In every instance the diagnosis of infectious ostemyelitis was made and antibiotic therapy was instituted to which the patient did not respond. More over all the cultures taken from the affected sites were negative for bacteria, fungi and mycobacteria, and all the immunological tests which were based on the probable diagnosis of chronic granulomatous disease or other immune deficiencies proved to be negative. After being diagnosed as a case of CRMO treatment with NSAIDs successfully controlled the disease process. In 7 months follow up no new exacerbation was noted.

Abstract: 

Introduction & Objective: The age related cataract surgery is most common ophthalmic surgery. Today the method of choice for cataract surgery is phacoemulsification. This study was conducted to compare the visual acuity, pre-operation and post operation ECCE & Phacemulsification complication in patients referring to Khalili & Oastgheib Hospital in Shiraz during 1380-1381. Materials & Methods: This prospective non randomized concurrent Control study was conducted on 100cases (52 case of ECCE and 48 case Pre-operative evaluation included age, sex, visual acuity, astigmatism according to keratometry difference and type of cataract. The patients were evaluated during the operation for size, site of incision, posterior capsular rupture, vitreous loss and nucleus dropped in vitreous. Then in 1, 5, 14, 90,180 days post operation, they were examined for BCVA, UNCVA, astigmatism (keratometry and refraction), Inflammation, Comeal edema, posterior capsular opacity and clinical CME. Results: In this study 100 cases (48 phacemulsification, 52 ECCE) aged 48-76 for cataract surgery were included. Patients were followed for 6 months after operation. Mean age in phacemulsification was 58.27 ±7.26 was 64.71±6.84. On the fifth day after ECCE, UNCVA≥30.60 was 15.3% and in phaco was 62.5%. On the fifth day mean UNCVA in phaco was 30.60 and in ECCE, 20.60. Mean astigmatism on the first day post-operation according to keratometry difference in ECCE was 4.84±2.08D and in phaco was 2.27±1.67D and according to refraction in ECCE was 3.62±2.08D and in phaco was1.91±1.34D. Posterior capsular rupture in ECCE was 7.6% and in phacor was 12.5%. Vitreous loss in ECCE was 5.7% and in phaco was 0.7%.Cornealedema in the first day post-operation in ECCE was 19.2% and in phaco was 43.8%. Two weeks after operation in all patients who underwent phacoemulsification, corneal edema was cleared but in ECCE 3.8% corneal edema continued for 6 months after operation. Nucleus dropped in vitreous occurred only in phaco (2.1%). lOL dislocation after  phacoemulsification was 4.1% and 2.1%(1 case) need to lOL removal but this complication was not visible in  ECCE. Conclusion: Success rate, Intra operative& post-operative complications in phacoemulsification in comparison to other studies were in an acceptable range. We recommend further study In this case for more documented results.
